database - Access form fill based on combobox -

i'm trying create booking form access database. have "id" combobox link following fields:

forename surname address 1 address 2 postcode city date of birth

i have achieved link forename , surname fields including information in lookup of combo box , using "=[combo33].[column](x)" control source. id prefer not put entire lot of information in combo box.

i'd appreciate help.
